- Current Photo of the Child
- Child’s Birth Certificate
- Child’s Social Security Card or State-Assigned Number
- Child’s Current Immunization Record
- Court Order or Custody Documents (if applicable)
- Most Recent Report Card (if applicable)
- Proof of Residency Documentation (1)
- Current property tax bill with parent/guardian’s name and property address;
- Current rental or lease agreement with parent/guardian’s name, student name and address, as well as manager or owner’s name and telephone number.
- Documents related to the purchase of the residence with the parent/guardian’s name and property address.
- Mail dated within 60 days before the application date from the following resources:
- Social Security Administration.
- A Texas State government agency.
- Utility companies.
- Credit card bill.
- Financial institutions; including check or savings accounts.
- Insurance companies.
- State and Federal Revenue documents.
- Paycheck Information.
- For students under a Parental Child Safety Placement (PCSP) agreement, a DFPS-issued letter containing the placement address and required statutory language.
- Guardian ID
- 504 Plans and/or Individual Education Plan Documents as applicable
Required Documents for Registration

- Proof of Physical Examination (no more than 12 months prior to the start of school)
- 2 Proofs of Residency Documentation
- Current property tax bill r homestead exemption.
- Contract for purchase of home.
- Warranty deed or lease agreement.
- A utility bill or work order dated within the past 60 days of enrollment
- Current TECO electric bill (cannot accept water or phone/cable.
- Affidavit of Residency (if the student’s family is residing in the home of another individual, the following are required:
- Notarized Affidavit of Residency
- Proof of residency (See Above) with bills in the name of the person you are living with
- ID for the individual you are residing with
- Guardian ID
- 504 Plans and/or Individual Education Plan Documents as applicable

- Deed, mortgage, lease, current homeowner’s, or renter’s insurance declaration page, or current real property tax bill;
- A utility bill or receipt of utility installation issued with 90 days of enrollment;
- A paycheck or paystub issued to the parent or student within 90 days of the date of enrollment that includes the address of the parent’s or student’s primary residence;
- The most current available bank statement issued to the parents or student that includes the address of the parent’s or student’s primary residence;
- Documented affirmation of address of student’s parent(s) from district of residence where parent(s) currently resides;
- Notarized affirmation from parent(s) or student(s) if over 18-years of age of current residence address;
- USPS return receipt from certified letter sent to parent(s) by district of residence;
- Written confirmation from the Department of Job and Family Services of current address of the parent(s); or
- Written confirmation from a local law enforcement agency of the current address of the parent(s).

- Proof of residency (See Above) with bills in the name of the person you are living with
- ID for the individual you are residing with
