Consent Forms & Submission Process

Completing Your Annual Consent Forms

Families are required to complete SB 12 consent forms once per school year for each student. These forms allow you to indicate your preferences for services, activities, and surveys covered under the law.

Consent choices remain in effect for the full school year and can be updated if circumstances change.

Understanding Your Choices

Each section of the consent form allows you to make informed decisions based on your family’s preferences.

  • Health & Mental Health Services
    Selecting “yes” allows your child to receive applicable services at school. Selecting “no” means those services will not be provided unless required by law or in an emergency.
  • Clubs & Activities
    Some clubs or student organizations require explicit parent approval. You may be contacted with additional details if your child is interested in participating.
  • Surveys & Screeners
    Parents can review survey questions before consenting. Schools will notify families in advance when surveys are scheduled.
