Required Documents for Registration

• Current Photo of the Child
• Child’s Birth Certificate
• Child’s Social Security Card or State-Assigned Number
• Child’s Current Immunization Record
• Court Order or Custody Documents (if applicable)
• Most Recent Report Card (if applicable)
• Proof of Residency Documentation (1)
  • • Current property tax bill with parent/guardian’s name and property address; 
    • Current rental or lease agreement with parent/guardian’s name and address, as well as manager or owner’s name and telephone number; 
    • Documents related to the purchase of the residence with the parent/guardian’s name and property address; 
    • For students under a Parental Child Safety Placement (PCSP) agreement, a DFPS-issued letter containing the placement address and required statutory language; 
    • Mail dated within 60 days before the application date from the following sources: 
      • Social Security Administration. 
      • A Texas State government agency; 
      • Utility companies; 
      • Financial institutions; including checking or savings statements; 
      • Insurance companies; 
      • State and Federal Revenue documents; 
      • Paycheck information 

    • If the parent or legal guardian cannot provide evidence of residency because the parent or legal guardian is living with a relative or friend, a notarized statement by the relative or friend may be accepted by IDEA Public Schools with the following stipulation:

      • Notarized statement must state that the parent or legal guardian and child are living with the relative/friend;
      • Notarized statement must state the name of the relative or friend who is on the relative or friend’s proof of residence;
      • Notarized statement must state the same address of the relative or friend who is on the relative or friend’s proof of residence;
      • A copy of the relative or friend’s proof of residence must be attached to the notarized statement (meeting the documentation criteria described above); and
      • The notarized statement must be signed by the same name of relative or friend who is on the relative or friend’s proof of residence.
      • Note: SIS Coordinators, Registrars, Enrollment Coordinators, APOs cannot notarize these documents
      • Scholars identified as McKinney Vento, Foster Care, or has an active-duty member of the armed forces follow additional protocols
• Guardian ID
• 504 Plans and/or Individual Education Plan Documents as applicable

• Proof of Physical Examination (no more than 12 months prior to the start of school)
• 2 Proofs of Residency Documentation
  • Current property tax bill r homestead exemption. 
  • Contract for purchase of home.
  • Warranty deed or lease agreement.
  • A utility bill or work order dated within the past 60 days of enrollment
  • Current TECO electric bill (cannot accept water or phone/cable.
  • Affidavit of Residency (if the student’s family is residing in the home of another individual, the following are required:
    • Notarized Affidavit of Residency
    • Proof of residency (See Above) with bills in the name of the person you are living with
    • ID for the individual you are residing with

• Guardian ID
• 504 Plans and/or Individual Education Plan Documents as applicable

  • Deed, mortgage, lease, current homeowner’s, or renter’s insurance declaration page, or current real property tax bill;  
  • A utility bill or receipt of utility installation issued with 90 days of enrollment;
  • A paycheck or paystub issued to the parent or student within 90 days of the date of enrollment that includes the address of the parent’s or student’s primary residence;
  • The most current available bank statement issued to the parents or student that includes the address of the parent’s or student’s primary residence;
  • Documented affirmation of address of student’s parent(s) from district of residence where parent(s) currently resides; 
  • Notarized affirmation from parent(s) or student(s) if over 18-years of age of current residence address; 
  • USPS return receipt from certified letter sent to parent(s) by district of residence;
  • Written confirmation from the Department of Job and Family Services of current address of the parent(s); or
  • Written confirmation from a local law enforcement agency of the current address of the parent(s).
  • Affidavit of Residency (if the student’s family is residing in the home of another individual, the following are required:
    • Notarized Affidavit of Residency
    • Proof of residency (See Above) with bills in the name of the person you are living with
    • ID for the individual you are residing with
• Guardian ID
